Telmisartan is marketed in the United States to treat high blood pressure.
Telmisartan blocks the action of angiotensin II (Ang II), a major efferent molecule in the renin-angiotensin-aldosterone system (RAAS). This is the sixth such Sartan to hit the market after the lead compound losartan. Its lasting effect (24 hour half-life) may be the main difference from other angiotensin II antagonists. Unlike several other drugs in this category, its activity is not dependent on conversion to an active metabolite, 1-o-acylglucosidic acid being the main metabolite found in the human body. Telmisartan is an effective competitive antagonist of the AT1 receptor that mediates most of the important effects of angiotensin II while lacking affinity for the AT2 subtype or other receptors involved in cardiovascular regulation. In several clinical studies, once daily dose of telmisartan produced effective and sustained hypertensive effects with a low incidence of side effects
Drug interaction:
Anesthetic: enhances the antihypertensive effect.
Analgesics: antagonize antihypertensive effects and increase the risk of renal impairment with NSaids; Hyperkalemia caused by ketorolac and other non-steroidal anti-inflammatory drugs.
Antihypertensive drugs: Use of ACE-Is and aligilen is associated with an increased risk of hyperkalemia, hypotension, and renal impairment.
Cardioside: digox-in concentration increased.
Cyclosporine: Increased risk of hyperkalemia and renal toxicity.
Diuretics: enhance the antihypertensive effect; Hyperkalemia and potassium-preserving diuretics.
ESAs: Increased risk of hyperkalemia; Antagonistic effect of antihypertensive action.
Lithium: Reduced excretion (increased likelihood of lithium toxicity).
Potassium salt: Increased risk of hyperkalemia.
Tacrolimus: Increased risk of hyperkalemia and renal toxicity.
